Community Health & Wellness for Tribal Citizens

 

Health Education and Prevention services are provided by the Community Health Staff who include a Community Health Nurse (CHN), a Community Health Representative (CHR) and a Health Educator.  Together they provide and coordinate a number of health services that include health screenings, infectious disease education and prevention, chronic disease management, safety education and planning, and injury prevention. 

Top priority wellness issues include increasing healthy eating habits, increasing physical activity and decreasing tobacco use.  Community Health staff design Tribal programs to address these issues and to provide support for citizens working to improve their health.  Health interactions may be provided on an individual basis or in a group event and occur in a variety of settings such including the Tribal Community Center, Tribal Health offices, the Jamestown Family Health Clinic, or during a visit to the home of a Tribal citizen.

The following programs are provided under special grants:

The Women, Infant and Child (WIC) program provides supplemental foods for pregnant mothers and families with children up to the age of five.  Community Health staff help parents learn about their child’s development and to provide a helping hand with child-raising concerns.  Dietary Consulting Services are available from a Registered Dietician.

A monthly Diabetes Lunch and Support Group provides an opportunity for the Tribal Community to get together, socialize and learn more about diabetes prevention and care.  Community Health staff provide individual support during home visits to help teach families and individuals to monitor their health and manage their care. 

The Injury Prevention and Safety program includes purchasing and child car seats and bicycle helmets to Tribal families.  The CHR demonstrates the proper installation and use of these items.  The CHR also helps Elders and other Tribal members identify health and safety concerns within the home.

The Nicotine Cessation Program’s Community Health Nurse (CHN) can work with you and your provider to provide nicotine replacement therapy (NRT) for up to 10 weeks.  This program is available to the Tribal Community whether enrolled or not.

The Washington Breast & Cervical Health Program (WBCHP) provides education and information on where to obtain breast and cervical exams for un-insured or under-insured women between the ages of 40-64 in the Tribal Community, whether enrolled or not.
